සෞඛ්‍ය ප්‍රවර්ධන කාර්යාංශය විසින් දිස්ත්‍රික් මට්ටමින් සපුරා ගත යුතු අවම සෞඛ්‍ය අධ්‍යාපන සහ සෞඛ්‍ය ප්‍රවර්ධන ඉලක්ක හයක් හඳුනාගෙන ඇත. මෙම ඉලක්ක අතරට පහත සඳහන් සෞඛ්‍ය ප්‍රවර්ධන සැකසුම් පහ සහ ප්‍රජා වේදිකාව ස්ථාපිත කිරීම සහ නඩත්තු කිරීම ඇතුළත් වේ:
සෑම වසරකම මහජන සෞඛ්‍ය පරීක්ෂක ප්‍රදේශයකට අවම වශයෙන් එක් නව ක්‍රියාකාරී සතුටු ගම්මාන සැකසුමක් ස්ථාපිත කර නඩත්තු කරනු ලැබේ. අලුතින් ස්ථාපිත කරන ලද සහ දැනටමත් ස්ථාපිත කර ඇති සතුටු ගම්මාන සැකසුම් දෙකම ඇතුළත් වේ. මෙම ඉලක්කය සෑම වසර දෙකකට වරක් ක්‍රමයෙන් වෙනස් විය හැකි අතර, ඒ පිළිබඳව ලිඛිතව MOOH වෙත දැනුම් දෙනු ලැබේ.
සෞඛ්‍ය වෛද්‍ය නිලධාරී ප්‍රදේශයකට වසරකට අවම වශයෙන් නව ක්‍රියාකාරී පෙර පාසල් සෞඛ්‍ය ප්‍රවර්ධන සැකසුම් පහක් ස්ථාපිත කර නඩත්තු කරනු ලැබේ (ප්‍රාදේශීය මට්ටමේ මුල් ළමාවිය සංවර්ධන නිලධාරී ප්‍රදේශයට අනුකූලව). බොහෝ ප්‍රදේශවල, එක් ප්‍රාදේශීය ECDO ප්‍රදේශයක් එක් MOH ප්‍රදේශයක් සමඟ පෙළ ගැසේ.
පවුල් සෞඛ්‍ය කාර්යාංශය (2015) විසින් එවන ලද පාසල් සෞඛ්‍ය ප්‍රවර්ධන චක්‍රලේඛය අනුගමනය කළ යුතු අතර WHO අග්නිදිග ආසියානු කලාපය (2021) විසින් සෞඛ්‍ය ප්‍රවර්ධන පාසල් ක්‍රියාත්මක කිරීම පුනර්ජීවනය කිරීම සඳහා වූ අන්තර් අමාත්‍යාංශ රැස්වීමේ දී ඇති කැපවීමට අනුව. ඒ අනුව, සියලුම පාසල් සෞඛ්‍ය ප්‍රවර්ධනය කළ යුතුය.
සෑම වසරකම සෞඛ්‍ය වෛද්‍ය නිලධාරී ප්‍රදේශයකට අවම වශයෙන් එක් නව ක්‍රියාකාරී සෞඛ්‍ය ප්‍රවර්ධන සේවා ස්ථානයක් ස්ථාපිත කර පවත්වාගෙන යනු ලැබේ. ප්‍රගතිය අනුව මෙය සෑම වසර දෙකකට වරක් ක්‍රමයෙන් වෙනස් විය හැකිය. එවැනි අවස්ථාවන්හිදී සෞඛ්‍ය වෛද්‍ය නිලධාරීවරයාට ලිඛිතව දැනුම් දෙනු ලැබේ.
සෑම දිස්ත්‍රික්කයකටම අවම වශයෙන් එක් නව ක්‍රියාකාරී සෞඛ්‍ය ප්‍රවර්ධන රෝහලක් වාර්ෂිකව ස්ථාපිත කර අඛණ්ඩව පවත්වාගෙන යනු ලැබේ.
සෑම වසරකම PHM ප්‍රදේශයකට අවම වශයෙන් එක් ක්‍රියාකාරී MSG එකක් ස්ථාපිත කර නඩත්තු කරනු ලැබේ. අලුතින් ස්ථාපිත කරන ලද හෝ දැනටමත් ස්ථාපිත කර ඇති කණ්ඩායම් මෙම දර්ශකයට ඇතුළත් වේ. ප්‍රගතිය අනුව මෙය ක්‍රමයෙන් වෙනස් විය හැකිය. එවැනි අවස්ථාවන්හිදී MOOH වෙත ලිඛිතව දැනුම් දෙනු ලැබේ.
Health Education and Health Promotion targets aim to reach the objectives of HPB collectively.
Targets are the benchmarks which are the desired change expected for achieving the objectives. Several targets will be responsible for achievement of an objective. The Key Performance Indicators are what we measure to assess progress towards the achievement of targets and thereby the objectives. Therefore, several KPI’s will progress towards achievement of a target.

Key Performance Indicators of Health Education & Health Promotion activities
Following KPI’s can be used at district level to monitor performance in achieving the HE &HP targets. The formulas to calculate each indicator are given in Annexure 4. The KPII from 1 to 7 are indicators on establishment of settings. These will directly showcase achievement of HE & HP targets. Several advocacy, mediation and enabling sessions will be needed to establish and maintain the functionality of a health promotion settings. The KPII from 8 to 13 will showcase progress of strengthening communication, health promotion, and health education. The KPII 14 to 17 indicate the progress of supportive supervision, monitoring and evaluation of HE&HP activities in the district.
